Social Worker, MSW- SCS Home Health- Lacey, Washington
Schedule: Full Time/ Days
Responsible for social work evaluation of patient/family psych/social and environmental needs, as well as implementation of the social work treatment plan and documentation consistent with insurance guidelines. Work is evaluated by Home Health Manager through patient record documentation, verbal communication and direct observation.

Required Qualifications:
Master's Degree: MSW from a school of Social Work.
Upon hire: Washington Advanced Social Worker License or Washington Advanced Social Worker Associate License or Washington Clinical Independent Social Worker License or Washington Clinical Independent Social Worker Associate License
1 year experience in a health care setting.
3 years Post Master's work experience, or prior experience in healthcare care setting.
