Immediate opening is available for a Director of Social Work.

The Social Worker is responsible for directing and overall management of the psychosocial need of the residents. Must be able to provide counselling or seek psychiatry experts for patient’s well being and improvements. Must be able to work independently and collaboratively as part of an interdisciplinary team. Delegation of authority

Authority is delegated to the individual in this position to:

• Review policies and procedures to provide social services to residents in compliance with federal, state and local regulations.

• Interview, assess and develop individualized plans to meet residents’ needs.

• Assess the facility’s social service program and make recommendations to the administrator.

Requirements

Qualifications:

  • Candidate with Master in Social Work (MSW). Nursing home experience is a must.
  • At least 2 years experience in Long Term Care setting
  • In lieu of a Master's degree, bachelors degree in Social Work with 5 years of experience in Long Term Care Setting
  • Must be flexible with schedule
  • Effective Communicator, ability to resolve complaints and seek resolutions to concerns
Requirements:

Duties include but are not limited to:

  1. Completes admission assessments
  2. Interview of new patients
  3. Complete MDS sections as required
  4. Complete ASCEND and Level of Care (LOC) for SNF eligibility
  5. Complete careplans as needed
  6. Resident ongoing support: conduct resident 72-hour meetings, enter notes and care plan
  7. Ensure mistreatment allegation and grievances are properly investigated
  8. Discharge planning - Ensure that living arrangement changes are ascertained prior to patient discharge
  9. Financial issues – meet and discuss with patient and responsible parties
  10. Family Crisis – intervene on behalf of patients
  11. Provide counselling for other significant events that may affect patients
  12. Upon admission submit ASCEND to proper agencies
  13. Assist residents with getting level of care extensions
  14. Work with Business Office Manager to assist long term care residents with benefit approval for long term stay
