This position is part of the Care-A-Van mobile care unit. This person will be visiting different clinics in the Richmond Market.

Primary Function/General Purpose of Position

The Community Health Worker is a trusted member of the localities in which they work. The primary role of the CHW is to serve as the liaisons between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. CHWs also build individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support, and advocacy. The CHW is responsible for assisting individuals and communities to adopt healthy behaviors. Conduct outreach for Care-A-Van personnel and Bon Secours to implement programs in the community that promote, maintain, and improve individual and community health. May provide information on available resources, provide social support and informal counseling, advocate for individuals and community health needs. May collect data to help identify community health needs.

Employment Qualifications

  • Certification as Community Health Worker is preferred.
  • Working knowledge of Virginia Child Health Insurance Program (FAMIS), the Child and Adult Medicaid, and the local Departments of Social Services' programs, resources, regulations and requirements for categorical assistance programs is preferred.
  • Previous work in health care, outreach work, community health, case management, and social work preferred.
  • Must be bilingual (English/Spanish)
  • Knowledge of the social determinants of health and their impact on health is preferred.
  • Knowledge of basic case management principles and practices preferred
  • Ability to prioritize, organize and complete daily work and assigned tasks both independently and with team members with minimal direct supervision.
  • Exceptional skills in public speaking with the ability to represent Bon Secours Richmond Health System locally, regionally, and nationally.
  • Previous work experience and/or training in a not-for-profit organization is desirable.
  • Experience working with underserved populations is desirable.
  • Ability to deal with patients in crisis situations, and know when to refer to a provider for mental health assistance.
  • Ability to work remotely is required

Essential Job Functions
  • Outreach Support:
    • Responsible for outreach both in the community and with local partners and businesses including schools, churches, and stores.
    • Conduct home visits for high-risk individuals to monitor their progress or assess their needs.
    • Contact clients in person, by phone, or in writing to ensure they have completed required or recommended actions.
    • Distribute flyers, brochures, or other informational or educational documents to inform members of a targeted community.
    • Interpret, translate, or provide cultural mediation related to health services or information for community members.
    • Refer community members to needed health services.
    • Assist families to apply for social services, including Medicaid.
    • Develop plans or formal contracts for individuals, families, or community groups to improve overall health.
    • Knows and builds relationships with local community members and leaders to determine health needs of the community and to build trust.

  • Community education and advocacy:
    • Serves as a community connection point for convening community residents around public health programs, research interventions, and community assessments.
    • Advocates for community change through participation in City and Statewide initiatives.
Advise clients or community groups on issues related to their care

  • Community health navigation:
    • Follow up with neighborhood residents and at risk patients in the clinic to connect them to local medical homes and social services through phone calls, clinic visits, letters, and occasionally home visits.
    • Work with clients to make sure they are attending their scheduled medical visits, understand their medications, and are checking health signs regularly (BP, etc).
    • Make sure clients have appropriate health coverage (Medicaid, Plan First, VCC, etc.) and educate clients on ways to avoid going to the ED.
    • Connect clients to other local resources they may need (ex: food, clothing, shelter, education, jobs, GED training, etc.). Document all client interactions in an online database.
    • Establish partnerships with existing Bon Secours staff, community social workers, and other community workers.

  • Customer Service:
    • Consistently provides courteous, friendly, and helpful services to all customers.
    • Arrives to work on time and keeps appointments on time.
    • Uses available resources to answer customer questions accurately, thoroughly, completely, and in a timely manner.
    • Maintain updated client records with plans, notes, appropriate forms, or related information.

V. Working Conditions

1. Works in multiple community settings including offices, Care-A-Vans, schools, clinics, homes, churches, and hospitals, up to a 50-mile radius of the City of Richmond, Must provide own transportation.

2. Travels between clinic sites in all weather conditions

3. The individual performing this job may reasonable anticipate coming into contact with human blood and other potential infectious materials. Individuals in this position are required to exercise universal precautions, use personal protective equipment and devices, and learn the policies concerning infection control. Some lifting of small children may be required.
