Council on Aging of Southwestern Ohio (COA) is looking for Licensed Social Workers and community-based RNs to help support the aging population, individuals with disabilities, and caregivers. Your duties will include: - Providing support/education to clients with complicated social/behavioral/environmental issues
- Conducting home visits
- Utilizing clinical judgment and critical thinking skills to facilitate appropriate client physical and behavioral healthcare and coordinating psychosocial and medical wraparound services to promote effective utilization of available resources
- Reporting specific health and social information back to the integrated team to assist in development of care/service plan
- Advanced care planning with client and others involved with client care
- Authorizing services in the most efficient means possible to meet identified client needs
Qualifications: - Ohio LSW or RN licensure
- At least one year of experience working with persons with disabilities/chronic conditions and LTSS.
- Prior or current experience with mental/behavioral health populations
- Prior experience completing electronic documentation
- Prior case management experience and experience working in a program with strict and multiple contractual requirements is a plus
- Critical thinking skills
- Ability to effectively use clinical judgment
- Ability to learn in-person, through a written format and virtually
- Have above average technology skills
- Ability to work in multiple software systems simultaneously
- A good working knowledge and understanding of issues related to the elderly population.
- Knowledge of community resources within Southwestern Ohio.
- Good oral and written communication skills
- A valid driver's license and reliable transportation for travel and conducting home visits and attending trainings
- Proficient computer skills including the Windows operating systems, MS Word, MS Excel, data entry and the ability to learn new software applications as needed
